The form should be filled out by the founder
as far, as possible. Most people in Germany
cannot go back in history earlier than til the
end of the 30-jears war (1618-1648)
because most files are burned at that time.
Only (usually aristocratic) families, who
have their own archives, fell not into
oblivion and can find their roots far into the
earliest mediaeval times. You can download
